Your card will be charged the day of your appointment. If you are a No Show, you will be charged a NO Show fee of $75. We do understand that life happens, so if you need to reschedule your appointment, please give a 24 hour notice to avoid a fee.
What is Charm EHR?
Charm is an online HIPAA compliant Electronic Health Records system. PHR stands for Personal Health Record (PHR) portal and is used to manage your health information. You can store your current and past medications, lab results, record health vitals and track progress using the built-in reports. You will go through Charm to fill out your New Patient Forms.

How do I log in to Charm PHR?
The first time you log into Charm, you will need to access the site through an email invitation sent from MeehanMD. The email will be from MeehanMD with "Patient Portal Registration" in the subject line. If you can't find it, check your spam folder. Once you have followed the email link, you will verify your identity with your date of birth. Select the correct year, month, and date from the calendar and click next. If you see an error message, email [email protected] to verify we have your correct birthday in our records. After you have entered your date of birth, you will be able to create a login ID and password to use in the future.

How do I share documents with the clinic from my patient portal?
To share DOCUMENTS, you will need to upload the document the SHARE the individual document by hovering over the SHARE tab to the right of the document. If you do not share the document then it will not be visible to any of our team members.
Select the Care Member from the drop down menu.
Document(s) will be shared to Practice and selected members will be notified.
Select the appropriate staff member.
